Continuum of Care

1. Continuum of Care

Outreach, intake, and evaluation to identify care and accommodation needs and provide a link to the required level of both.

2. Shelter

Assist with providing emergency shelter and a safe environment as a quick and secure alternative to avoid sleeping on the streets.

3. Accommodation

Transitional accommodation and social services to help people build the skills they’ll need until they’re permanently housed.

4. Stabililty

Affordable permanent housing and/or permanent supportive housing provided to individuals and their families with the purpose of providing permanent relief.

Distinct forms of homelessness: crisis, episodic, and long-term.

Crisis Homelessness

One episode of homelessness that lasts longer than a year is referred to as crisis homelessness.

Episodic Homelessness

The second or third episode of homelessness, lasting longer than a year, is referred to as episodic homelessness.

Long-term Homelessness

Long-term homelessness is described as four or more episodes of homelessness in 3 years or a current episode of homelessness that has lasted longer than a year.

Community Homes and Restoration Management Services (CHARMS)

While Community Homes and Restoration Management Services (CHARMS) looks to address all three forms of homelessness, resolving long-term homelessness is the main priority for the non-profit 501(c) 3 corporation. Those who are homeless for a long time need a lot of public assistance, such as temporary housing, healthcare, mental health counseling, child welfare, foster care, and rehabilitation support.
